Leaders Report Form

There are two leader report forms.  You must have Adobe Acrobat Reader  to view and print the form for use at the home group meetings.  This form is for your information only and allows for one months worth of weekly meetings.  If your home group meets differently please fill in for the weeks your group meets. 

At the end of each quarter please fill out the Quarterly Report Form and submit it to Home Fellowship Leaders Int.  We would like to know how the home group is doing and offer any support we can.  It is not difficult and should take less then 5 minutes.

Information on these forms will be used to help us better meet the needs of home groups and improve our lesson plans.  We would appreciate knowing what your victories and prayer needs are so we can support you better.
